Nominees

If you purchased, sold, or held Envision common stock during the period from and including August 10, 2018, through and including October 11, 2018 (the “Settlement Class Period”), and held the shares in your name, you are both the beneficial holder, purchaser, or acquirer of the stock and the record holder, purchaser, or acquirer of the stock. If, however, you held, purchased, or acquired Envision common stock during the Settlement Class Period and the shares were registered in the name of a third party, such as a nominee or brokerage firm, you are the beneficial holder, purchaser, or acquirer of the stock, but not the record holder, purchaser, or acquirer of the stock. The third party is the record holder, purchaser, or acquirer of the stock, then within fifteen (15) calendar days of November 9, 2020, you must either:

  1. Provide the Claims Administrator with a list of the names and last known addresses of the beneficial owners described above; or
  2. Request from the Claims Administrator sufficient copies of the Notice and Claim Form (“Notice Packet”) to forward to all such beneficial owners and, within fifteen (15) calendar days of receipt of those copies, forward the Notice Packet to such beneficial owners.

If you are providing a list of names and addresses to the Claims Administrator:

  1. Compile a list of names and addresses of the beneficial owners that purchased, sold, or held Envision common stock during the period from and including August 10, 2018, through and including October 11, 2018 (the “Settlement Class Period”), and held the shares in your name, you are both the beneficial holder, purchaser, or acquirer of the stock and the record holder, purchaser, or acquirer of the stock.
  2. Prepare the list in Microsoft Excel format. A preformatted spreadsheet can be found by clicking here.
  3. Then you must do one of the following:

    1. Burn the Microsoft Excel file(s) to a CD or DVD and mail the CD or DVD to:

    2. Email the spreadsheet to info@envisionmergersettlement.com; or
    3. Upload the spreadsheet here; or

If you are mailing the Notice Packet to beneficial owners:

Request the needed number of copies of the Notice Packet via email to info@envisionmergersettlement.com. You must mail the Notice Packets to the beneficial owners within ten (15) calendar days of your receipt of the Notice Packets.
